Transcripts and Student Records

Requests for student records, including high school transcripts, must be made in writing.

Please include the following information:

  • Name attended under
  • Year of graduation or dates of attendance
  • Date of birth
  • Current name and address

Mail your request to:

Or you can send your request, via e-mail, to studentrecords@liverpool.k12.ny.us

If you submit an e-mail request, your transcript will be sent directly to your college or university, the  human resources department at your place of employment, or your union training center. To have a transcript sent to a home address, the Student Records Office must receive a written letter with your signature and witnessed by a notary.
