NYS Makes Changes to Health Forms for Student Physicals

Dear Liverpool CSD Parents & Guardians,

As you are aware, Education Law requires that all New York State (NYS) public school students have a health exam when they are a new student in a school district and when they enter Pre-K or Kindergarten, and grades 1, 3, 5, 7, 9, and 11.

Beginning January 31, schools can no longer accept a student’s health exam if it is not on the state’s required form or the required health record equivalent.

Click here for a copy of the required form with instructions for your health care provider (HCP). The form also is posted on the LCSD Web site at www.liverpool.k12.ny.us.

Please share the attached papers at your child’s next visit for a health exam with your health care provider (HCP). This is typically your doctor but may be a nurse practitioner or physician assistant.
